HUBUNGAN ANTARA KECEMASAN, DEPRESI, DAN STRES DENGAN KUALITAS TIDUR MAHASISWA FAKULTAS KEDOKTERAN DI INDONESIA SELAMA PANDEMI COVID-19
Abstract
Pendahuluan: Kualitas tidur merupakan wujud pemenuhan kepuasan seorang individu yang menjalani suatu proses tidur. Pandemi COVID-19 mengharuskan pemberlakuan social distancing sebagai salah satu pengendalian dalam penyebaran virus. Namun, hal tersebut dapat berdampak pada kesejahteraan psikologis dan bahkan dapat memengaruhi kualitas tidur seseorang. Mahasiswa, khususnya di fakultas kedokteran, rentan mengalami tidur dengan kualitas yang buruk. Faktor kecemasan, depresi, dan stres diduga ikut terlibat dalam kualitas tidur di kalangan mahasiswa kedokteran. Tinjauan pustaka ini bertujuan untuk mengetahui apakah tingkat depresi, kecemasan, dan stres berkontribusi terhadap kualitas tidur yang buruk pada mahasiswa kedokteran di Indonesia selama pandemi COVID-19
Pembahasan: Lima studi menyimpulkan bahwa terdapat hubungan tingkat kecemasan dengan kualitas tidur mahasiswa kedokteran selama pandemi COVID-19. Hal ini diduga karena gangguan tidur dan kecemasan sama-sama didasari oleh patogenesis hiperaktivitas yang disebabkan oleh gangguan sistem neurotransmiter, seperti kolinergik dan GABA. Dua studi meninjau hubungan antara kualitas tidur mahasiswa fakultas kedokteran dengan kejadian depresi selama masa pandemi COVID-19. Walaupun hubungan antara kualitas tidur dan depresi telah dibuktikan secara ekstensif, arah dari hubungan ini masih belum bisa ditetapkan. Mekanisme perubahan kualitas tidur pada depresi diduga melibatkan peran dari peningkatan mediator inflamasi seperti IL-6 dan TNF, jalur biokimia neurotransmitter, korelasi genetik, serta disregulasi pada irama sirkadian akibat single nucleotide polymorphism (SNP) dari clock genes. Tiga studi menemukan adanya hubungan antara kejadian stress pada mahasiswa kedokteran dengan kualitas tidur mereka selama pandemi COVID-19. Stres dan tidur memiliki keterkaitan dengan aktivasi aksis hipotalamo-hipofisis-adrenal (HPA) yang juga dipengaruhi oleh sistem imun.
Simpulan: Berdasarkan tinjauan pustaka yang dilakukan, ditemukan bahwa kualitas tidur memiliki berkaitan erat dengan kejadian stres, depresi, dan kecemasan pada mahasiswa fakultas kedokteran di Indonesia selama pandemi COVID-19.
